What this service is

This sign-in records your attendance at Sentry MS for a drug test, inspection, or prize claim. It is provided by La Familia Dynamics LLC on their behalf. Using it means you accept these terms and the Privacy Policy.

Accurate information

The details you give — including your name, date of birth, and licence numbers — must be your own and must be accurate. Your licence photographs must be of your own valid licences. Signing in on behalf of someone else, or giving false details, may invalidate your test and may be reported.

Your licence details are read automatically from the photographs you take. You are shown what was read before you submit, and it is your responsibility to correct anything wrong.

Fees

Where a fee applies, it is $50.00 paid in cash or $55.00 paid by credit card. The difference reflects card processing costs. Whether a fee applies to your visit depends on the arrangement between Sentry MS and the company you drive for; some companies are billed directly and you pay nothing on the day.

Refunds

If it is established that you did not need a test but a test was carried out anyway, you will be refunded in full. Contact [email protected] with the date of your visit and your licence number, and the refund will be issued by the method you paid, within 14 days.

Fees are not otherwise refundable once a test has been carried out — including where a result is one you did not expect, since the fee pays for the test being performed rather than for any particular outcome.

Test results

This sign-in records attendance only; it does not produce, hold, or report your test result. Results are handled by the testing provider and by Sentry MS under the rules that apply to them. Questions about a result should go to them, not through this sign-in.

Availability

The service is provided as-is. It depends on a working camera and internet connection, and may be unavailable at times. If it is unavailable, staff on site can record your visit another way — you will not be turned away because this page did not load.

Limits

To the extent the law allows, La Familia Dynamics LLC is not liable for indirect or consequential loss arising from use of this sign-in, and total liability is limited to the fee you paid for the visit concerned. Nothing here limits liability that cannot lawfully be limited.

Governing law

These terms are governed by the laws of the State of New York, and the courts of New York have jurisdiction.

Changes

These terms may change. The version in force is the one shown here on the date you sign in, and the date it was last changed is at the top of this page.
