This notice explains what is collected when you sign in for a drug test, inspection, or prize claim, why it is collected, and who receives it.
Who handles your information
The sign-in is operated by La Familia Dynamics LLC on behalf of Sentry MS, who decides what is collected and why, and who holds the records afterwards. Questions about your information, or requests to see or correct it, should go to [email protected].
What is collected
- Your name and date of birth
- Your TLC licence number and your DMV licence number
- A photograph of your TLC licence and of your DMV licence
- A photograph of your face, but only if you ask for an NYSFTD ID card to be printed — it is the picture printed on that card
- Your phone number and email address
- Your signature
- The company you drive for, your reason for visiting, whether you drive for another broker, your chosen language, which location you signed in at, and the date and time
Why it is collected
To confirm who attended, to meet drug and alcohol testing record-keeping obligations, and to match your visit to your testing record. The licence photographs are how your identity is verified and evidenced.
Automatic reading of your licences
When you photograph a licence, the image is sent to Google Cloud Vision to read the text on it, so the licence number and your details can be filled in without typing. Google processes the image to provide that service. You can correct anything it reads incorrectly before you submit.
Who it is shared with
- Sentry MS — the completed record, including your licence photographs, is written to their records and emailed to them
- Google — Cloud Vision to read your licences, and Google Sheets to store the record
- Otherwise it is not sold, rented, or shared. There is no advertising, and no analytics or tracking of any kind on this site.
How long it is kept
Records and photographs are kept for one year from the date of your visit, after which they are deleted. Copies held by Sentry MS in their own records are kept according to their own schedule and legal obligations.
How it is protected
Information is sent over an encrypted connection. Licence photographs are not publicly accessible: they can only be opened through a signed link or by a signed-in administrator, and are never cached by content delivery networks or search engines. Backups are encrypted.
Your choices
You may ask what is held about you, ask for corrections, or ask for your information to be deleted, by contacting [email protected]. Deletion may not be possible where a record must be retained to meet a legal or regulatory obligation — testing records are typically in that category.
Providing this information is voluntary, but it is required in order to be tested or inspected: without it the visit cannot be recorded.
